U.S. High School Transcript Assessment Form
When you completed the CCCApply application, you provided De Anza with your U.S. high
school transcript information. Most students earn their English or ESL and math course
placements based on the assessment of information.
To view your placement, log in to MyPortal and open the Apps section, then click on the Student Registration tile. When the registration landing page opens, look under Student Records and click on View My Placement Results.
If you don't see a placement, you'll need to submit the U.S. High School Transcript Assessment Form to the Assessment Center. Click the button to download the form, then fill it out and email it to daassessment@deanza.edu

GED Score Assessment Form
De Anza accepts the General Education Diploma (GED) Fifth Series test scores (2014-present)* for English and Math course placement. If you'd like us to assess your GED scores, here is what you should do

- Email it to daassessment@deanza.edu

*If your GED scores were earned before 2014, we strongly recommend that you consider completing the online guided self-placement (GSP) assessments for English and Math. These are not tests--they are online surveys--that will help you to determine which English and Math courses are best for you to start with.
If you're interested in taking ESL courses, you'll want to complete the ESL assessment process.

2006 California Chemistry Diagnostic Exam Score Transfer Form
A score of 22 or higher out of 44 on the 2006 California Chemistry Diagnostic Exam is required to earn placement into CHEM1A and BIO6A.
